§1149.44. What must documents filed with the ALJ include?

45 C.F.R. § 1149.44

(a)

Documents filed with the ALJ must include:

(1)

An original; and

(2)

Two copies.

(b)

Every document filed in the proceeding must contain:

(1)

A title, for example, “motion to quash subpoena”;

(2)

A caption setting forth the title of the action; and

(3)

The case number assigned by the ALJ.

(c)

Every document must be signed by the filer, or his/her representative, and contain the address or telephone number of that person.
