The principle place of business and offices of the agency are located at 230 South 500 East, Suite 230, Salt Lake City, Utah 84102-2045. All correspondence and requests for information or other materials should be submitted to the agency at this address.
The Executive Director is the agency official designated to accept service of process on behalf of the agency.
