§500.211. Contents of notice.
29 C.F.R. § 500.211
The notice required by § 500.210 shall:
Set forth the determination of the Secretary and the reason or reasons therefor.
Set forth, in the case of a civil money penalty assessment—
A description of each violation; and
The amount assessed for each violation.
Set forth the right to request a hearing on such determination.
Inform any affected person or persons that in the absence of a timely request for a hearing, the determination of the Secretary shall become final and unappealable.
Set forth the time and method for requesting a hearing, and the procedures relating thereto, as set forth in § 500.212.
