§102.12. Contents.

29 C.F.R. § 102.12

(a)
A charge must contain the following:
(1)
The full name and address of the person making the charge.
(2)
If the charge is filed by a labor organization, the full name and address of any national or international labor organization of which it is an affiliate or constituent unit.
(3)
The full name and address of the person against whom the charge is made (referred to as the Charged Party).
(4)
A brief statement of the conduct constituting the alleged unfair labor practices affecting commerce.
(b)
Attachments to charges are not permitted.
