§175.130. What information must I provide when I request service?
25 C.F.R. § 175.130
At a minimum, you must provide the utility with the following information when you request service:
Your full legal name or the legal name of the entity needing service;
Your taxpayer identification number;
Your billing address;
Your service address; and
Any additional information required by the utility.
