(g) After a petition has been provisionally approved, a completed visa application form, any supporting documents required pursuant to §§ 42.63 and 42.65, and any required fees must be submitted to the consular officer in accordance with § 42.61 for a provisional review of visa eligibility. The requirements in §§ 42.62, 42.64, 42.66 and 42.67 shall also be satisfied to the extent practicable.

(b) A machine-readable visa shall be in the format designated by the Department, and contain, at a minimum, the following data:(1) Full name of the immigrant;(2) Visa symbol;(3) Location of the visa issuing office;(4) Passport number;(5) The registration number (A-number) assigned to the immigrant;(6) Sex;(7) Date of birth;(8) Nationality;(9) Date of issuance;(10) Date of expiration;(11) Visa control number;(12) Any annotations entered to reflect waivers or other information useful to an immigration officer at a port of entry (POE) upon the immigrant's application for admission to the United States;(13) A digitized photo of the immigrant; and

(a) Every alien applying for an immigrant visa, including an alien whose application is executed by another person pursuant to § 42.63(a)(2), shall be required to appear personally before a consular officer for the execution of the application or, if in Taiwan, before a designated officer of the American Institute in Taiwan, except that the personal appearance of any child under the age of 14 may be waived at the officer's discretion.
