§1261.305. Form of claim.
14 C.F.R. § 1261.305
The official designated in § 1261.308 shall, prior to acting on a claim, require the claimant to submit a completed Standard Form 95—“Claim for Damage, Injury or Death.”
NASA installations will furnish copies of Standard Form 95 upon request.
