# §254c–9. Establishment of program of grants

- (a) **In general—** The [Secretary](/usc/42/201.md?p=c) of Health and Human [Services](/usc/42/201.md?p=a) shall in accordance with sections 254c–9 to [254c–13](/usc/42/254c–13.md) of this title make grants to provide for [projects](/usc/42/13641.md?p=2) for the establishment, operation, and coordination of effective and cost-efficient systems for the delivery of essential [services](/usc/42/201.md?p=a) to individuals with lupus and their families.
- (b) **Recipients of grants—** A grant under [subsection (a)](#a) may be made to an entity only if the entity is a public or [nonprofit](/usc/42/300s–3.md?p=3) private entity, which may include a [State](/usc/42/201.md?p=f) or local government; a public or [nonprofit](/usc/42/300s–3.md?p=3) private [hospital](/usc/42/300s–3.md?p=1), community-based organization, hospice, ambulatory care facility, community health center, migrant health center, or homeless health center; or other appropriate public or [nonprofit](/usc/42/300s–3.md?p=3) private entity.
- (c) **Certain activities—** To the extent practicable and appropriate, the [Secretary](/usc/42/201.md?p=c) shall ensure that [projects](/usc/42/13641.md?p=2) under [subsection (a)](#a) provide [services](/usc/42/201.md?p=a) for the diagnosis and disease management of lupus. Activities that the [Secretary](/usc/42/201.md?p=c) may authorize for such [projects](/usc/42/13641.md?p=2) may also include the following:
  - (1) Delivering or enhancing outpatient, ambulatory, and home-based health and support [services](/usc/42/201.md?p=a), including case management and comprehensive [treatment](/usc/42/11851.md?p=11) [services](/usc/42/201.md?p=a), for individuals with lupus; and delivering or enhancing support [services](/usc/42/201.md?p=a) for their families.
  - (2) Delivering or enhancing inpatient care management [services](/usc/42/201.md?p=a) that prevent unnecessary hospitalization or that expedite discharge, as medically appropriate, from inpatient facilities of individuals with lupus.
  - (3) Improving the quality, availability, and organization of health care and support [services](/usc/42/201.md?p=a) (including transportation [services](/usc/42/201.md?p=a), attendant care, homemaker [services](/usc/42/201.md?p=a), day or respite care, and providing counseling on financial assistance and insurance) for individuals with lupus and support [services](/usc/42/201.md?p=a) for their families.
- (d) **Integration with other programs—** To the extent practicable and appropriate, the [Secretary](/usc/42/201.md?p=c) shall integrate the [program](/usc/42/274l–1.md?p=4) under sections 254c–9 to [254c–13](/usc/42/254c–13.md) of this title with other grant [programs](/usc/42/274l–1.md?p=4) carried out by the [Secretary](/usc/42/201.md?p=c), including the [program](/usc/42/274l–1.md?p=4) under [section 254b of this title](/usc/42/254b.md).
