# §15094. Application


To be eligible to receive a grant under this subchapter, a [State](/usc/42/15092.md?p=a-5) shall submit an application to the [Secretary](/usc/42/15092.md?p=a-4) at such time, in such manner, and containing such information and assurances as the [Secretary](/usc/42/15092.md?p=a-4) may require, including information about the designation of a lead entity, a description of available [State](/usc/42/15092.md?p=a-5) resources, and assurances that [systems change activities](/usc/42/15092.md?p=a-6) will be [family](/usc/42/15092.md?p=a-2-A)-centered and [family](/usc/42/15092.md?p=a-2-A)-directed.
